- Fix a passport photograph in your relocation approval form.
- Then, you write the following; Name, call up no, new state code, old state code, Acct name, Acct number, Bank name, Batch and phone no.

To all newly relocated corps members, Kindly be informed that you’re expected to bring along to the nysc secretariat the following documents:
- Two colored passport photograph
- Relocation Approval Form (color)
- Personel Data Form
- Nysc ID Card (photocopy)
Below are optional but important documents to bring along to avoid unnecessary drama includes;
- Statement of Result (photocopy)
- Medical License (for medical graduate)
- International Passport (for foreign graduate)
- Physical Verification Slip (for foreign graduate)
